What causes this fault?

This fault occurs when the boiler's control system detects that the flame has failed to stay lit or is burning unsafely after the heating has been triggered. The most common reason is a worn-out sensor that can no longer 'see' the flame, or a faulty gas valve that isn't providing the correct amount of fuel to maintain the burner. Because the boiler cannot guarantee a safe and consistent flame, it shuts down the system as a protective measure.
